For patients with no insurance:
You are responsible for all accepted services performed in our office. Payment, in full, is expected at the time of the visit unless other arrangements are made in advance.

Payment plans can be set up with CareCredit or Citi Health Card. These are funding companies that offer no-interest loans of 6 and 12 months for dental services. Please click on the links below to visit them online for more information, or call our office and we can assist with the application process. For services greater than $1500, we offer a 5% courtesy for pre-payment in advance with cash or check.
For patients with insurance:
You are responsible for all accepted services performed in our office. It is your responsibility to provide our office with your most current insurance information. Please be sure to notify us of any changes in your coverage. Our office is out of network with all plans. Once your insurance coverage has been verified, we will allow the insurance to pay our office directly for services. An estimate of your co-pay is expected at the time of the visit.
We may ask for a credit card to keep on file in our HIPAA-secure file to balance out any balance left unpaid by your insurance. Overpayment refunds can be charged back to this credit card.
Delta Dental Plans and, as of January 1, 2014, all Blue Cross and Blue Shield Plans will no longer send payments to our office. You will need to pay in full and claims will be filed electronically, and Delta and BCBS will pay you directly.
